For this form, there are three tasks that you will need to complete:

1. Fill out the application form.
2. Request for approval from a Direct Manager or Supervisor.
3. Request for approval from a Faculty Dean/Director.

After filling out your application, please fill out details for your two approvers. In most cases, you will be required to request for approval from these two different recommenders. In some cases, you may only require approval from a department head or similar. In the event that both of these signoffs are coming from the same person, please enter their information in each task (both need to be completed in order to submit).

After submission, your application will be reviewed by the LT Hub team, and if any further information is needed, you will be notified.
